Directions

Using PDF:

1.  Click on the link above to retrieve the .pdf file.
2.  Print out the form.  
3.  Fill out the form in its entirety - print or type clearly.
4.  Sign the completed form. 
5.  Send the signed application to District President for signature and letter. 

Using Word Document:

1.  Click on the link above to retrieve the .doc file.
2.  When document opens in new window, choose "Save File As." 
3.  Save the file to your computer. 
4.  Open the file and proceed to fill out the form in Microsoft Word. 
5.  Print out the completed form.
6.  Sign the completed form. 
7.  Send the signed application to District President for signature and letter.
